mp4's no good after update

This is the second time I have dealt with this. The first time, I completely reinstalled our server.

After doing an update, I can no longer build a working mp4 - thanks a lot! Any suggestions?

We need more information:-

As a minimum - Source video/image type and resolution, mp4 output settings, version and patch status of MEP.
